Draft social care data standards: feedback block one

This page contains the first block of draft social care data standards, which are now open for feedback.

We've organised the standards into use cases, which reflect different situations where information needs to be recorded and shared. Each use case contains a number of proposed data items, together with suggested names and definitions.

We'd like you to review the draft standards and tell us whether they're clear, reflect practice and contain the information needed to support the sharing of information across social care.

This first block includes:

  • front door – information about referrals to social care, such as a person's initial or subsequent contact with the front door to social care services
  • hospital discharge – information needed to support a person’s safe and timely discharge from hospital into an appropriate care setting
  • reablement – information needed to identify people who could benefit from reablement and support them to access a short-term reablement service
  • case closure - information that needs to be recorded when a person’s social care record is formally closed.

Open feedback form

Use our online form to record your feedback as you review the draft data standards below.

You can also download a copy of the form at the bottom of this page if you'd prefer to work offline.

Front door

These tables set out the proposed data items, names and definitions for the front door use case. The use case includes information about referrals to social care, such as a person's initial or subsequent contact with social care services.

Each table below sets out:

  • ID - the unique code we've given each data item. You can use this code when providing feedback so we know which data item you're referring to
  • Name - the proposed name of the data item (such as given name, family name or date of birth)
  • Definition - the proposed definition of the data item
  • Values - where relevant, the options that can be used for the data item (for example, 'yes' or 'no', or a list of local authorities). For some data items, we've linked to an external source of options rather than listing all available values.

We want your feedback on the proposed data items, their names and definitions.

All of the use cases we're asking for your feedback on will also include information about the person, their relations, carers and referrers. This includes things like name, date of birth, gender and ethnicity.

Because of this, we've collected all of these data items together in their own 'demographics' use case, which we'll ask for feedback on as part of a later block. We've listed the demographics data items relevant to this use case in the dropdown below for reference.
