Reshaping adult services in Powys through local hubs
The Locality Hub project is a transformation initiative aimed at reshaping adult services across Powys.
 
					The project finder is for people who lead, develop and deliver social care services who want to know more about work that’s happening across Wales.
We’ve created the project finder to help spread ideas and learn from practice by sharing information about the work, where it’s happening, who’s involved and how to find out more.
It aims to help people connect and learn from each other. We’ve encouraged people to share the important lessons they’ve learnt on the way, which could be really useful inspiration and learning for others.
We’ve included a collection of innovative practice examples and research initiatives that are relevant to social care in Wales. We’ve also featured academic research projects that we’ve formally supported through our Research Support and Engagement Framework.
Use the project finder to search for information about innovative practice examples and social care research initiatives, and to contact the lead person directly to find out more about their project, work together or support each other.

This service provides support to people living with late-stage dementia in Swansea and Neath Port Talbot and their carers to help prevent escalation of need and crisis resulting in admission to hospital or a care home.
People Speak Up is a community-based social, mental health, arts, health and well-being charity.
This project used the experience of care leavers and practitioners to produce a toolkit to support care leavers’ engagement with multi-agency services.
The Equilibrium Domestic Abuse Behaviour Change and Healthy Relationships Programme is an intervention programme for perpetrators of domestic abuse.
The North Wales Regional Innovation Coordination Hub (RIC Hub) coordinates research, innovation and improvement activity in North Wales around how health and social care services can improve the way they work together.
